Complete the registration form below to activate the warranty that's included with your CORENINE massage chair purchase.

In the MESSAGE section below, please include the following information:

1. Your home address.
2. The store name and location where you purchased massage chair.
3. The date you purchased chair.
4. Chair Model Number, e.g. C9000 (located on quick start guide).
5. Chair Serial Number (located on the back of the chair near the power switch or the front rail of the chair underneath the seat cushion)
